Agreement: By signature below, the Company hereby agrees to pay interest at the rate of 1.5% per month (18% per annum) or, if less, the maximum amount by applicable law, on all amounts past-due (1 or more days beyond the net due date), at Ecofolds Packaging Ltd. sole discretion, and further understands and agrees that if the Company’s account becomes past due, Ecofolds Packaging Ltd. may at any time hold and/or cancel orders and/or refuse to accept further orders and may accelerate all unpaid invoices so that they become due and payable immediately. Upon signing, it is certified that the information provided on this application/agreement is true and accurate. In the event of any court action by Ecofolds Packaging Ltd. against the Company, the Company agrees that the laws of the Province of Alberta will prevail. The Company further agrees that no oral modifications or agreements regarding this credit application/agreement will be effective. The Company hereby consents to the obtaining of credit information as may be required from time to time, about credit, hereby applied for or any renewal or extension thereof and to the disclosure of any credit information concerning the Company to any credit reporting agency or credit association. The Company hereby acknowledges that Ecofolds Packaging Ltd. further reserves the rights to increase or decrease any credit limit or terms without notice, as it may deem appropriate. Ecofolds Packaging Ltd. may revoke credit privileges upon written notification to the Company.

Privacy Policy and Consent: I acknowledge that Ecofolds Packaging Ltd. may have collected personal information from me, as defined by Personal Information protection and Electronics Documents Act or other provincial legislation.

At least one Officer or Authorized Signatory of the Company must sign this document.
